Consists in injecting one’s own fat into the breasts. This lipofilling technique is becoming increasingly popular with women seeking breast augmentation without implants.

In cases where fat is used exclusively, breast augmentation will be moderate, not exceeding a volume increase of more than one cup (a patient with an A cup can have a B cup).

The patient must have a sufficient reservoir of fat to be able to undergo this procedure, which excludes very thin patients.

Breast lipofilling can be used alone or in combination with breast implants.

THE PRE-OPERATIVE CONSULTATION:

This is essential in order to ascertain the patient’s wishes and to carry out a thorough questioning and clinical examination:

  • Learn about the patient’s medical, surgical and obstetrical history.
  • Examine the breasts and determine measurements and skin quality.
  • Perform a complete body examination and determine the fat harvesting areas.
  • Detailed explanation to the patient of the procedure and its after-effects.
  • Information on any complications arising from the procedure.
  • Request a biological and radiological assessment

THE OPERATION AND ITS AFTERMATH:

  • After consultation with the anaesthetist and viewing of the check-ups
  • Breast lipofilling is performed under general anesthesia and the procedure lasts an average of 2 hours.
  • Procedure: The surgeon begins by performing liposuction in areas where there are sufficient fat reserves (stomach, lower back, love handles, saddlebags, inner thighs and knees). The fat is then centrifuged (for 3 minutes) to separate the good-quality fat cells from the other components” fig1.
  • The surgeon makes micro-incisions (1 to 2 mm) in the submammary fold and distributes the fat evenly across the breasts using fine cannulas. Some of this fat (15 to 30%) will reabsorb over a period of one to three months.
  • At the end of the operation, the incisions are sutured with absorbable thread. A dressing is also applied by the surgeon. It is essential to wear a support bra day and night for 1 month.

Complications

  • INSUFFICIENT RESULTS : This is THE essential complication, and is due to resorption of a proportion of the fat injected. A second session can be envisaged at a later date (> 6 months) if the fat donor zones are still available.
  • INFECTION AND/OR FAT NECROSIS : This complication is very rare, occurring in less than 1% of cases. Treatment is often medical and sometimes surgical (drainage).
  • OILY CYSTS : Painless swelling caused by poor preparation of the injected fat. Good fat preparation helps prevent this.
